Patient Referral Coordinator

Assist in the transition of patients from the acute care setting to aftercare facilities, agencies or programs in the community in collaboration with the multidisciplinary team, payers and community providers. Provide information and assistance to internal and external customers on aftercare programs and facilities. Duties include coordination of paperwork and discharge documents, communication of requests for review and maintenance of reports and statistics pertaining to aftercare placement activities. Requirements: High School Diploma. Three years experience in a hospital setting or related health field. Continuing care or home care experience preferred.
The referral coordinator will be responsible for:
Accepting all incoming Endocrine and Bone Health referrals Contact patient to schedule consult appointment Utilize Allscripts worklist for referral communications and updates Confirm insurance referral is active if required Run insurance eligibility through Eligibility Tracker in APM or insurance website Closing the loop for quality and reporting measures Help with phones and various office responsibilities as needed Sign on Bonuses for Specialty rolesFlexible SchedulingPaid Time OffCommunity based hospitalFree ParkingFor union positions we also offer:
