Pharmacist PRN Opportunity at Heart Hospital of Austin

As a Pharmacist at Heart Hospital of Austin, you will play a crucial role in ensuring the appropriate and effective use of pharmaceuticals for patient care. Your expertise will be valued as you interact with patients and healthcare providers to define compliance with current standards of practice and assure continuity of services to achieve desired patient outcomes.
Responsibilities- Monitor and evaluate pharmaceutical use to make recommendations for improvement
- Interact with patients and healthcare providers to ensure compliance with current standards of practice
- Assure continuity of services to achieve desired patient outcomes
- Accurately interpret medication orders and transcribe to the computer system
- Provide medication therapy and accurately compound and dispense pharmaceuticals
- Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose
- Suggest appropriate, cost-effective therapeutic medication alternatives to medical staff
- Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges, and promote rational drug therapy selection
- Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs as defined by Pharmacy Leadership
- Complete notification reports regarding medication errors and adverse drug reactions
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD)
- Previous hospital experience preferred
- Current Pharmacist license with Texas State Board of Pharmacy (TSBP)
