Job Summary
Heart Hospital of Austin, a leading provider of healthcare services, is seeking a skilled Registered Nurse Case Manager to join their team. As a key member of the rehabilitation team, the successful candidate will be responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes.Responsibilities
The ideal candidate will provide program orientation to patients, families, and caregivers, promoting their participation in team discussions related to plans, goals, and status. They will also ensure the implementation of patient treatment plans, facilitating the involvement of patients throughout the rehabilitation process. Additionally, the successful candidate will document findings of discharge planning evaluations and psychosocial assessments, communicating social, financial, or discharge needs and preferences of patients and their families.Requirements
To be successful in this role, the candidate will require 1+ years' experience in case management, an Associate Degree in Nursing or Nursing Diploma, and current licensure as a Registered Nurse in the state(s) of practice. Certification in Case Management is preferred.About Heart Hospital of Austin
