LPN - Extended Care Specialist

We are seeking a skilled LPN to join our team at Providence Extended Care. As a collaborative member of our nursing team, you will provide high-quality care to our residents, working closely with our interdisciplinary healthcare team to ensure the best possible outcomes.
Key Responsibilities- Administer medications and treatments as prescribed by physicians
- Assist with care and interventions, including life engagement and supervision of team members
- Monitor implementation of care plans and coordinate referrals to other healthcare team members
- Provide education and support to residents and their families
- Graduate of an accredited School of Nursing as a Licensed Practical Nurse
- Alaska Licensed Practical Nurse upon hire
- Within 30 days of hire: National Provider BLS - American Heart Association
- Municipality of Anchorage food handler permit or ability to attain certification within 21 days of hire
- 6 months nursing experience in a healthcare facility (acute care, long term care, rehabilitation center)
- Supervision/leadership experience
