Position Overview

Role: Resident Care Coordinator
Type: Full-Time
Shift: Various, on-call for weekend coverage required
Compensation: $43,000/year

Location: Avamere at Oak Park

Key Responsibilities:

  • Document and monitor changes in residents' health status, needs, and appointments.
  • Ensure meticulous documentation of care plans, nursing notes, and all resident communications.
  • Possess strong experience as a Medication Aide.
  • Provide training and orientation for Caregivers and Medication Aides.
  • Assist residents with daily living activities, including bathing, dressing, and eating.
  • Measure, record, and assist in wellness clinics.
  • Conduct regular audits on a weekly, monthly, and quarterly basis.
  • Oversee the scheduling of clinical staff.
  • Foster an environment of warmth, patience, enthusiasm, and joy.
  • Maintain confidentiality of resident care in compliance with HIPAA regulations.
  • Engage in ongoing training as required.
  • Perform additional duties as assigned.

Qualifications:

  • High School diploma or equivalent.
  • Certification as a Nursing Assistant is preferred.
  • Minimum of 1 year of substantial experience as a Caregiver/Medication Aide in an Assisted Living setting.
  • Must obtain a Food Handlers Card within 30 days of employment.
  • Must secure First Aid & CPR Certification within 30 days of employment.
  • Proof of COVID-19 vaccination is preferred but not mandatory.
