Ambulatory Clinical Pharmacist

We are seeking a highly skilled and experienced Ambulatory Clinical Pharmacist to join our team at Children's National Hospital. As a key member of our pharmacy team, you will play a critical role in providing high-quality patient care and contributing to the development of our ambulatory clinical pharmacy services.
Key Responsibilities:- Conduct comprehensive medication reviews and interventions during care transitions to optimize medication therapy and improve clinical outcomes.
- Assist with medication reconciliation to reduce the risk of omissions and/or duplications.
- Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals.
- Collaborate with the Transitions of Care Team to ensure seamless transitions of care for patients.
- Participate in quality improvement initiatives to enhance patient safety and outcomes.
- Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
- Minimum 3 years of experience as a clinical pharmacist with involvement in medication management, drug use evaluation, and quality assurance activities.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
- Knowledge of Joint Commission accreditation requirements and pharmacy regulatory requirements.
