Ambulatory Clinical Pharmacist

This is an exciting opportunity to join the Division of Pharmacy Services at Children's National Hospital as an Ambulatory Clinical Pharmacist. In this role, you will play a critical part in improving medication safety, education, and adherence for our patients.
Key Responsibilities:- Conduct comprehensive medication reviews and interventions during care transitions to optimize medication therapy and improve clinical outcomes.
- Assist with medication reconciliation to reduce the risk of omissions and/or duplications.
- Update medication histories as appropriate and provide recommendations to providers to optimize medication therapy.
- Participate in Collaborative Practice opportunities in specialty clinics, pending DC requirements and credentialing at Children's National Hospital.
- Minimum 3 years of experience as a clinical pharmacist with involvement in medication management, drug use evaluation, and quality assurance activities.
- Ambulatory care experience is preferred, as is experience in a pediatric hospital.
- Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy is required.
