Job Summary

We are seeking a highly skilled and compassionate County Case Manager to join our team at SPWA Services. As a key member of our support coordination team, you will play a vital role in facilitating individuals with developmental disabilities and their families to locate and secure appropriate support services.

Responsibilities include developing personalized support plans, connecting clients with Medicaid-approved providers, and monitoring services to ensure effectiveness. Successful candidates will possess strong communication skills, be creative in utilizing community resources, and have excellent organizational abilities.

The ideal candidate will have a Bachelor's Degree, minimum of 1-year experience working with the I/DD community, and proficiency in written and verbal communication. Strong technological device skills, including online web interfaces, are also required. This is a part-time to full-time position requiring periodic travel within Burlington County and surrounding areas.
