About the Role

Eliot Community Human Services is committed to delivering exceptional care and support to adults with severe and persistent mental illnesses. We are seeking a dedicated Licensed RN to join our team as a Psychiatric Nurse Specialist - Community Case Manager.

The ideal candidate will possess strong knowledge of psychiatric medications, treatments, and case management principles. This role requires excellent communication and interpersonal skills, as well as the ability to work independently and as part of a collaborative team.

Responsibilities
  • Act as a primary case manager for 3-6 consumers, providing comprehensive assessments and developing individualized plans to promote recovery and rehabilitation.
  • Provide nursing interventions, including medication management and health education, to individuals in their homes or community settings.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective service delivery.
  • Participate in regular supervision, staff meetings, in-services, and trainings to maintain professional development and stay up-to-date on best practices.
  • Transport consumers to work sites, meetings, appointments, and activities as needed.

Requirements
  • Licensed Registered Nurse (RN) in Massachusetts
  • Strong knowledge of psychiatric medications, treatments, and case management principles
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a collaborative team
Compensation and Benefits
