Job Summary:
A Care Transitions Coordinator is sought by Eliot to join their Behavioral Health Division. This role involves working with adults with psychiatric disabilities, promoting mental health recovery and wellness.

About the Role:
This innovative Care Management approach focuses on individuals with complex medical, behavioral health, and social needs. As a member of the Care Transitions Team, you will collaborate with Nurse Care Managers to triage hospital admissions data and develop assertive follow-up strategies.

Responsibilities:
• Promote hope and empowerment through meetings and support of individuals
• Assist enrollees in identifying choices and developing self-advocacy skills
• Facilitate collaborative discharge planning for hospitalized enrollees during admissions
• Coordinate timely face-to-face contact with enrollees in the community following inpatient discharge

Requirements:
Bachelor's degree or master's degree in human services field; 3 years' direct experience in Care Management or Case Management; or Certified Peer Specialist or Certified Recovery Coach. May substitute one year of experience with a college degree in human services field if Peer Specialist or Recovery Coach.

Benefits:
$50,000 annual salary; 6% bilingual differential if candidate is fluent in Spanish (Bilingual Salary: $53,000 annually); Eliot is committed to dismantling barriers preventing people with marginalized racial identities from actualizing their full employment potential.
