Care Manager II Role

About the Job

We are seeking a skilled Care Manager II to join our team at Children's National Hospital. As a Care Manager II, you will be responsible for providing comprehensive care coordination services to members enrolled in our Health Services for Children with Special Needs (HSCSN) health plan.

Key Responsibilities

  • Assess member needs and develop, monitor, and evaluate treatment plan progress
  • Communicate with members on their assigned caseload to coordinate care and refer to appropriate services
  • Collaborate with healthcare providers and community resources to ensure seamless care delivery
  • Document member interactions and care coordination activities in a timely and accurate manner

Requirements

  • Master's degree in Social Work or related field
  • 3 years of experience in healthcare or managed care, with a focus on care management
  • Effective verbal and written communication skills
  • Bilingual Spanish speaking a plus
  • Licensed in DC as a Licensed Independent Social Worker (LICSW)

Work Environment

This is a full-time position with a 40-hour work schedule. The ideal candidate will be able to work independently and as part of a team to provide exceptional care to our members.
