Care Manager II

AmeriHealth Caritas is seeking a highly skilled Care Manager II to join our team. As a Care Manager II, you will be responsible for assisting members with complex medical needs in achieving their optimal level of health through self-management.
The ideal candidate will have a strong background in nursing or social work, with a minimum of 3 years of clinical practice experience working with adults and/or pediatrics with complex medical and behavioral health diagnoses. Bilingual (Spanish/English) is required, and a current and unrestricted Registered Nurse licensure in the District of Columbia and a Bachelor's Degree in Nursing or a Master's Degree in Social Work is necessary.
Key Responsibilities:
- Assess members to determine care coordination and case management needs
- Complete comprehensive person-centered assessments of physical health, psycho-social health, environmental, and social determinants of health and supportive needs
- Identify problems/barriers for care coordination and appropriate care management interventions
- Coordinate physical, behavioral health, and social services
- Provide medication management, including regular medication reconciliation and support of medication adherence
- Create a plan of care to assist members in reducing/resolving problems and or barriers so that members may achieve their optimal level of health

- A competitive salary range of $82,400 - $112,300
