Care Manager II, BSN/LICSW

We are seeking a highly skilled Care Manager II, BSN/LICSW to join our team at AmeriHealth Caritas. As a key member of our healthcare team, you will be responsible for providing exceptional care to our members, working closely with healthcare providers, and promoting healthy outcomes.
Responsibilities- Provide comprehensive care management services to members with complex medical and behavioral health diagnoses.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Conduct regular home visits and in-person assessments to ensure members receive necessary care and support.
- Coordinate with community resources and social services to address members' needs.
- Monitor and report on member outcomes, identifying areas for improvement and implementing changes as needed.
- Current and unrestricted Registered Nurse licensure in the District of Columbia and a Bachelor's Degree in Nursing.
- Current and unrestricted Licensed Independent Clinical Social Worker (LICSW) licensure in the District of Columbia and a Master's Degree in Social Work.
- Minimum of 3 years of clinical practice experience as a RN or LICSW working with adults and/or pediatrics with complex medical and behavioral health diagnoses.
- Minimum of 1 year of case management experience supporting Medicaid members within a managed care organization preferred.
- A competitive salary range of $78,600 to $107,100.
