About the Role

We are seeking a highly skilled and compassionate Prenatal Case Manager to join our team at Family Health Centers of San Diego. As a Prenatal Case Manager, you will play a critical role in supporting our patients' health and well-being by coordinating their care plans and ensuring they receive the necessary resources and services.

Key Responsibilities
  • Assist in patient flow and ensure that test results and referrals are accurately recorded and communicated to the appropriate team members.
  • Complete all required documentation accurately, thoroughly, and in a timely manner, in accordance with department standards.
  • Coordinate clients' individual care plans and oversee the completion of all recommendations made on the care plan.
  • Assist clients with practical arrangements such as translation needs, referrals, special appointments, and utilization of internal and external services.
  • Ensure cases are closed following established standardized protocols.
  • Keep prenatal logs current and provide appropriate and timely follow-up.
  • Perform other duties as assigned.
Requirements
  • 1 year of experience in perinatal or women's health care preferred.
  • Bachelor's degree in health science or social science field preferred.
  • High School diploma or GED equivalent required.
  • Medical Assistant diploma or LVN desirable.
  • Ability to accurately and quickly perform data entry.
  • Ability to work effectively with patients from diverse social, cultural, and economic groups.
  • Ability to work well in both a team-based environment and independently.
  • Bilingual in Spanish/English required.
  • Demonstrated ability to be culturally sensitive and respect diversity.
  • Good written and verbal communication skills.
  • Knowledge of perinatal programs, care management, and procedures.
  • Strong computer literacy ability to comply with department needs and expectations.
  • Strong interpersonal and customer service skills.
