POSITION OVERVIEW/RESPONSIBILITIES

In alignment with the Mama Bexar integrated maternal health initiative, this role will act as a crucial link between patients, healthcare facilities, and medical professionals to enhance maternal health outcomes.

This position will collaborate within a multidisciplinary care team to support and guide patients throughout their prenatal, delivery, and postpartum journeys. Responsibilities include facilitating access to screening services, providing education, and coordinating patient care.

Key duties involve ensuring that patients receive timely information, navigating them to medical appointments, and following up on their healthcare needs. All tasks and interactions are relevant to both clinical and community environments. Additional responsibilities will encompass patient tracking, data management, and reporting.

QUALIFICATIONS/EXPERIENCE

A Bachelor's degree in social sciences, health education, or a related discipline is required.

Alternatively, four years of relevant experience in health education, public health, social work, or case management may substitute for the Bachelor's degree.

Community Health Worker certification is preferred. Familiarity and experience with similar programs are advantageous. Proficiency in both English and Spanish is mandatory.
