Your Role as an Outpatient Substance Abuse Counselor:
As a Substance Abuse Counselor, you will play a crucial role in guiding our patients through their treatment and recovery journey from opioid use disorder.

Your responsibilities will include providing empathetic and effective care through both individual and group counseling sessions.


Key Responsibilities:
- Deliver compassionate and effective guidance in individual and group counseling settings.
- Plan, manage, and document patient recovery processes.
- Co-facilitate group or family therapy sessions as required.
- Ensure timely and clear documentation of patient care and treatment incidents.
- Develop personalized treatment plans for each patient.
- Conduct initial and follow-up assessments.
- Assess patient needs and determine necessary referrals to additional programs or facilities.
- Prepare aftercare plans for assigned patients as needed.
- Provide crisis intervention when necessary.
- Perform case management duties to ensure individualized quality care.
- Act as a liaison between patients and referral sources as required.

Qualifications:
Education and Skills:
- Bachelor's degree in a social or health services field from an accredited institution is required.
- Bilingual proficiency in English and Spanish is highly preferred.
- Previous experience in addiction recovery and/or medication-assisted treatment environments is beneficial.

Licenses and Certifications:
- LCDP certification is preferred but not mandatory.
