Patient Scheduler Role

We are seeking a Patient Scheduler to join our team at NHC HomeCare Franklin. As a Patient Scheduler, you will be responsible for coordinating admissions, evaluations, resumption of services, and discharges, as well as documenting these events with the relevant disciplines.

Responsibilities:
  • Facilitate daily schedules by entering scheduling changes in a timely manner
  • Notify patients, caregivers, and other stakeholders of scheduling changes
  • Coordinate patient admissions, evaluations, and discharges
  • Document all patient-related activities and changes
Requirements:
  • High school diploma or equivalent
  • Two years of experience in home care or a related field
  • Excellent communication and customer service skills
  • Ability to work in a fast-paced environment
  • Reliable transportation
