Homecare Specialist Position

NHC HomeCare Franklin is seeking a highly motivated and detail-oriented individual to join our team as a Homecare Specialist. In this role, you will be responsible for facilitating patient care through coordination of scheduling, admissions, and discharges.

Duties:
  • Enter scheduling changes in a timely manner
  • Notify patients, caregivers, and other stakeholders of scheduling changes
  • Coordinate patient admissions, evaluations, and discharges
  • Document all patient-related activities and changes
Requirements:
  • High school diploma or equivalent
  • Two years of experience in home care or a related field
  • Excellent communication and customer service skills
  • Ability to work in a fast-paced environment
  • Reliable transportation
Compensation:
  • Salary: $55,000 - $70,000 per year
