Registered Nurse for Admissions at Capital Caring

The Admissions Registered Nurse (RN) plays a crucial role in coordinating hospital-based hospice and palliative care services. This position is integral to ensuring that patients receive timely and compassionate care tailored to their needs.
Key Responsibilities
- The Admission RN is responsible for managing all hospice and palliative care referrals, facilitating admissions to the appropriate services.
- Engage with patients and their families, as well as the attending physician, to ensure seamless communication and care planning.
- Triage incoming referrals and collaborate with interdisciplinary team members to address the immediate needs of patients.
- In partnership with the hospice and palliative care medical consultant, initiate care plans and discharge planning, while keeping the interdisciplinary team informed about patient goals and treatment strategies.
- Conduct nursing assessments and make informed treatment decisions based on patient and family preferences, in alignment with professional standards.
- Foster relationships with acute care facilities to promote hospice programs and provide opportunities for professional development and community education.
Supervisory Duties
The Admissions RN supervises LPNs, CNAs, Unit Secretaries, and Volunteers, ensuring a high standard of care and support within the team.
Qualifications
- A minimum of two years of experience in public health, community health, medical-surgical nursing, or a related field is required. Experience in home care, hospice, or oncology nursing is preferred.
- Candidates must possess at least a Diploma or Associate's degree in Nursing from an accredited program; a Bachelor of Science in Nursing (BSN) is preferred.
- Current licensure from the appropriate governing body for practice in the relevant jurisdictions is required, along with Healthcare Provider BLS (CPR) certification.
