Hospice Clinical Liaison Job Summary

The Hospice Clinical Liaison serves as the primary point of contact for hospitalized patients and their families, introducing them to hospice and palliative care services. This role requires strong communication and interpersonal skills to educate patients and their families about hospice care and provide ongoing support.

Key Responsibilities:

Initiates the plan of care, discharge plan, and communicates with the interdisciplinary team regarding patient goals and treatment plans.
Makes nursing assessments, judgments, and treatment decisions based on patient needs and wishes.
Promotes Capital Caring and Capital Palliative Care Consultants programs to acute care facilities through collaborative relationships with the facility, providing opportunities for professional development and community education.

Requirements:

Must have one year of experience in a comparable setting/unit, with hospice and/or palliative care experience strongly desired.
Must have a degree in Nursing from an accredited nursing program, with a BSN degree desired.
Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from a compact state is acceptable.
Must have a driver's license in good standing and access to a vehicle during working hours.
