Nurse Care Specialist

Job Summary
We are seeking a skilled Nurse Care Specialist to join our team at Capital Caring Health. As a Nurse Care Specialist, you will provide high-quality patient care in the comfort of their own homes or facilities.
Key Responsibilities
- Provide nursing assessments and develop care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure seamless care coordination
- Make timely visits to patients with emergent or critical needs
- Arrange for, teach, and supervise nursing care in home care and facility settings
Requirements
- One year of staff nurse experience in a comparable setting/unit
- Hospice and/or palliative care experience is strongly preferred
- Licensure from the governing body of practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia
- Healthcare Provider BLS (CPR) certification
