Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at Adara Home Health. As a Private Duty RN, you will provide high-quality, client-centered care to individuals in their homes, promoting independence and dignity.

Key Responsibilities
  • Plan, provide, and evaluate nursing care in accordance with established policies and procedures.
  • Recognize and interpret symptoms, and institute remedial measures within the legal limits of Licensed Practical Nursing in the state of Minnesota.
  • Document observations and teachings completely, accurately, legibly, and concisely.
  • Administer medications and treatments as ordered by the physician and delegated by the Case Manager.
  • Initiate rehabilitative nursing procedure techniques as appropriate.
  • Exercise professional judgment in adapting nursing measures to changing needs.
  • Communicate effectively with all members of the interdisciplinary team.
  • Maintain accurate clinical records and prepare clinical and progress notes.
  • Assist clients in arranging medical appointments and contacts with community resources.
  • Maintain a safe client environment and identify and report any accidents, injuries, or suspected vulnerable client abuse, neglect, or financial exploitation.
Requirements
  • Currently licensed as an RN or LPN in the state of Minnesota.
  • At least one year of experience in a healthcare setting, with acute hospital or home care experience preferred.
  • Demonstrates ability to make appropriate nursing judgments.
  • Strong interpersonal communication and teaching skills.
  • Strong organizational skills and the ability to work independently.
  • Excellent written and oral communication skills.
  • Proof of negative mantoux or documentation of negative chest x-ray.
  • Current CPR certification highly recommended.
  • Access to a dependable vehicle or public transportation to travel to multiple company business stops.
  • A valid driver's license and proof of car insurance when using a personal vehicle for company business.
  • U.S. Citizenship or evidence of valid Alien Work Permit.
  • Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services.
  • Pass initial and ongoing background studies and screenings.
