The Registered Nurse is responsible for delivering comprehensive care to clients as designated by the Case Manager, adhering to the Nurse Practice Act and the company's established policies and procedures.

This role involves providing both physical and emotional support, as well as educating clients and their families under the guidance of a Registered Nurse Case Manager. The RN will report directly to the Clinical Supervisor.

KEY RESPONSIBILITIES

  • Develops, implements, and assesses nursing care in line with established protocols, consulting with the Case Manager for any necessary adjustments.
  • Identifies and interprets symptoms, taking appropriate actions within the legal scope of nursing practice and promptly reporting significant changes in client conditions.
  • Accurately documents observations and educational interactions.
  • Administers medications and treatments as prescribed by physicians and delegated by the Case Manager, in accordance with the Nurse Practice Act.
  • Initiates rehabilitative nursing procedures as needed.
  • Uses professional judgment to adapt nursing interventions to meet changing client needs.
  • Communicates effectively with all members of the healthcare team.
  • Participates in client conferences as required.
  • Maintains precise clinical records, including clinical notes and timely documentation.
  • Assists clients in scheduling medical appointments and accessing community resources as directed.
  • Ensures a safe environment for clients, reporting any incidents or suspected abuse or neglect.
  • Takes responsibility for personal professional development through ongoing education and training.
  • Upholds confidentiality regarding all client and employee information.
  • Promotes a positive image of client and employee services.
  • Adapts to diverse family dynamics and home settings.
  • Exercises discretion in accepting assignments and performs additional duties as assigned.
  • Participates in after-hours on-call processes to address client care and staffing needs.
PHYSICAL AND ENVIRONMENTAL REQUIREMENTS

  • Medium to Very Heavy workload, requiring physical transfers and lifting of clients. Care is provided in individual homes, necessitating adaptability to various environments.
  • May require exerting over 100 pounds of force occasionally, over 50 pounds frequently, and over 20 pounds constantly to move objects. Physical demands exceed those typical for heavy work.

QUALIFICATIONS

  • Current RN or LPN licensure in Minnesota.
  • License must be active and free from revocation or restrictions.
  • Minimum of one (1) year experience in a healthcare setting, preferably in acute care or home health.
  • Demonstrated ability to make sound nursing judgments.
  • Strong communication and teaching skills.
  • Excellent organizational skills and capability to work independently.
  • Proficient written and verbal communication skills.
  • Proof of negative mantoux or negative chest x-ray documentation.
  • Current CPR certification is highly recommended.
  • Access to reliable transportation for travel to various locations.
  • Valid driver's license and proof of insurance required for personal vehicle use.
  • Must disclose any criminal history related to healthcare services. Candidates with relevant convictions will not be considered.
  • Must pass initial and ongoing background checks, including those from the Minnesota Department of Health.
