Job Summary

We are seeking a highly skilled and compassionate Social Worker-Coordinator to join our team at Enriching Lives. The successful candidate will be responsible for overseeing the services provided to individuals with developmental disabilities, developing and implementing individualized service plans, and ensuring the highest level of quality care.

Key Responsibilities
  • Develop and implement individualized service plans in collaboration with the planning team
  • Ensure the provision of medical, health, and ancillary services to individuals in accordance with their service plans
  • Arrange and facilitate opportunities for relationship development and community activities based on individual preferences
  • Monitor and support family home providers during home visits and meetings
  • Provide crisis management support and rotating on-call coverage to individuals and providers
  • Develop and maintain working relationships with individuals, families, funding sources, and regulatory agencies
Requirements
  • Related bachelor's degree (B.A.) from a four-year college or university plus one to two years of related experience
  • Experience working with individuals with developmental disabilities
  • Excellent computer skills and familiarity with Microsoft Office programs
  • Ability to read, write, and speak effectively
