Social Services Coordinator

Merrimack County Nursing Home is seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing social services to our nursing home residents.
Key Responsibilities- Provide social services to nursing home residents, including counseling, support, and advocacy.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support.
- Conduct assessments and evaluations to identify resident needs and develop strategies for improvement.
- Provide education and support to residents and their families on various topics, including health, wellness, and community resources.
To be successful in this role, you will need:
- A Bachelor's Degree (Master's Degree preferred) in social services, counseling, or a related field.
- Two years of experience in social work or a related field.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Strong problem-solving and critical thinking skills.
