Job Summary:

The Clinical Services Liaison for Complex Disease Management will serve as the primary liaison between patients and their medical teams, advocating for patients with a focus on heart failure and COPD.

This role requires a strong understanding of clinical care and a commitment to delivering exceptional patient experiences.

Responsibilities:

  • Develop and maintain strong relationships with patients and their families
  • Provide education and support to patients on disease management and self-care
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Identify and address patient needs and concerns

Requirements:

  • High School Diploma or equivalent required; Bachelor's degree preferred
  • Three to five years of nursing experience required; experience in care of targeted patient population preferred
  • RN licensure required
