Job Description:


At Archbold Medical Center, we are seeking a skilled Clinical Care Coordinator to join our team. As a registered nurse, you will play a vital role in coordinating patient care, ensuring seamless transitions between healthcare services, and promoting optimal health outcomes.



Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Coordinate with community resources, healthcare services, and social support systems to ensure comprehensive care.
  • Monitor patient progress, identify areas for improvement, and make necessary adjustments to care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure clear understanding of needs and goals.
  • Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes.


Requirements:

  • Current Georgia RN license.
  • Previous experience in caring for chronic disease patients.
  • 3-5 years of experience in a clinical or community health setting.
