Job Summary

We are seeking a highly skilled Medical Assistant Integrated Care Coordinator to join our team at WellSpace Health. As a key member of our care team, you will play a vital role in providing high-quality, patient-centered care to our clients.

The Medical Assistant Integrated Care Coordinator will work closely with our healthcare providers to coordinate patient care, provide education and support, and facilitate communication between patients, families, and healthcare teams.

Responsibilities
  • Provide medical assistance and support to patients, including vital sign collection, medication administration, and room preparation.
  • Coordinate patient care, including scheduling appointments, communicating with healthcare providers, and facilitating referrals.
  • Provide education and support to patients and families, including health education, disease management, and wellness promotion.
  • Facilitate communication between patients, families, and healthcare teams, including phone calls, emails, and in-person meetings.
  • Assist with data collection, reporting, and analysis, including patient outcomes, quality metrics, and program evaluation.
  • Participate in quality improvement initiatives, including process improvement, patient safety, and risk management.
  • Collaborate with interdisciplinary teams, including healthcare providers, social workers, and other support staff.
  • Maintain accurate and up-to-date patient records, including medical history, treatment plans, and progress notes.
  • Comply with all relevant laws, regulations, and policies, including HIPAA, OSHA, and Joint Commission standards.
Requirements
  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  • Current CPR or CPR/AED certification required.
  • Experience working in a healthcare setting, preferably in a medical assistant or care coordination role.
  • Excellent communication and interpersonal skills, including verbal and written communication, active listening, and conflict resolution.
  • Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
  • Strong analytical and problem-solving skills, including data analysis, problem identification, and solution development.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Flexibility to work varied shifts, including evenings, weekends, and holidays.
