Job Description

Job Summary

Hospice Buffalo is seeking a skilled Dietitian to join our interdisciplinary team. As a Dietitian, you will provide nutritional counseling to patients and their families, addressing nutritional issues related to health or disease states. You will work collaboratively with the healthcare team to develop and implement individualized patient care plans.

Responsibilities

  • Provide skilled nutrition services to patients and families in accordance with Hospice Buffalo's policies and procedures, as well as state and federal guidelines.
  • Participate in Interdisciplinary Group/Team (IDG) meetings, providing active incorporation of the nutrition needs of the patient within the plan of care and updates the plan of care when appropriate.
  • Assess and reassess nutrition factors to help patients and families cope with problems in daily living and to help them follow medical recommendations to maximize patient's quality of life.
  • Instruct and advise patients, family, and other IDG members in the principles of good nutrition, including food selection and preparation alternatives and supplements to enhance the patient's quality of life.
  • Assist the physician and other members of the hospice team to understand the significance of nutrition and emotional factors related to the client's case.
  • Participate in patient conferences as appropriate, demonstrating sound management of issues presented.
  • Demonstrate appropriate infection control procedures, including sharps disposal/hand hygiene, waste disposal, and use of personal protective equipment.
  • Adhere to requirements of both Hospice & Palliative Care Buffalo, Inc. and Hospice Buffalo, Inc. policies and procedures that are both patient-focused (clinical) and organizational-focused (administrative). Adhere to The Health Insurance Portability and Accountability Act (HIPPA) regulations, policies, and standards of confidentiality.
  • Collaborate with other agencies and professionals to facilitate patient/family care in a timely fashion.

Qualifications

  • Education/Certification
  • Bachelor's degree program in food and nutrition from a college or university approved by the education department that is also registered by the American Dietetic Association or the equivalent of such training, experience, and active participation in continuing dietetic education.
  • Registration and membership in the American Dietetic Association (ADA) preferred.
  • Valid Driver's License, free of serious violations, and vehicle required for use when on duty.

Experience

  • Minimum of 2 years of experience required; Hospice and/or home care experience preferred.
