Key Responsibilities

The Clinical Services Supervisor is accountable for overseeing and coordinating clinical operations, ensuring the delivery of nursing care to patients in their residences as prescribed by healthcare providers and in compliance with relevant laws, regulations, and organizational policies.

Responsibilities include:
  • Receiving patient referrals and ensuring appropriate clinician assignments for timely evaluations.
  • Coordinating patient home health benefits, medical necessity determinations, and ongoing insurance approvals.
  • Continuously assessing patient needs and ensuring that care is tailored to individual requirements, appropriate, and compliant with physician directives.
  • Overseeing the development, implementation, and updates of individualized patient care plans.
  • Managing communications with physicians, clinicians, patients, and referral sources, documenting updates and new orders as necessary.
  • Reviewing assessments and care plans daily, consulting with clinicians for recommendations as needed.
  • Coordinating care across various disciplines, including physicians, medical equipment providers, caregivers, and other healthcare professionals.
  • Following up on laboratory and diagnostic test results, ensuring timely physician notifications and necessary care plan adjustments.
  • Facilitating case conferences and ensuring effective information exchange among disciplines.
  • Assisting in the transfer and discharge processes for patients as indicated by healthcare providers.
  • Documenting incidents and infections in the Risk Management Incident Reporting System as per policy.
  • Providing guidance and leadership to clinical team members in collaboration with the clinical director.
  • Participating in quality assurance and performance improvement programs.
Qualifications

Education & Experience:
  • Current RN licensure in the state of practice.
  • Current CPR certification required.
  • Valid Driver's License and access to reliable transportation.
