Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelliChoice Home Care. As a LPN, you will provide one-on-one nursing care in the home to critical, complex clients focusing on trach and/or vent patients of all ages.

Responsibilities:
  • Provide high-quality, patient-centered care in the home setting
  • Work closely with patients and their families to develop and implement individualized care plans
  • Collaborate with our respiratory therapists to provide comprehensive care and support
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements:
  • Current LPN license
  • CPR certification (in-person training only)
  • TB skin test (current within last 12 months)
