Job Description:

We are seeking a skilled Licensed Practical Nurse to join our team at a Skilled Nursing Facility. As an LPN, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Responsibilities:

  • Perform designated job duties under the direction and supervision of a Registered Nurse while maintaining the LPN scope of practice.
  • Participate in the development of an individualized interdisciplinary plan of care.
  • Implement the individualized interdisciplinary plan of care according to biophysical, psycho-social, and age-specific needs.
  • Monitor patient/resident outcomes and report abnormal findings to the Assistant Clinical Manager/Supervisor.
  • Administer specified medications according to established standards and policies.
  • Document patient's status according to the unit-specific procedure.
  • Maintain educational requirements as required by the North Carolina Board of Nursing.
  • Documents according to federal and state regulations.
  • Perform other duties as assigned while maintaining the LPN scope of practice.

Requirements:

  • Completion of an accredited practical nursing program.
  • A minimum of one (1) year of experience in Long Term Care Facility is required.
