About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Consulate Health Care. As an LPN, you will play a vital role in providing high-quality nursing care to our residents, collaborating with our healthcare team to implement individualized treatment plans, and supervising and evaluating Certified Nursing Assistants (CNAs) to ensure they are delivering care to the highest standards.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each resident
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication
  • Supervise and evaluate CNAs, providing feedback and coaching to ensure they are meeting our high standards
  • Participate in fall-risk and pressure ulcer prevention programs, actively working to prevent and manage these conditions
  • Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary
Requirements
  • Active license to practice as an LPN
  • Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred
