Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans, taking into account the unique needs and goals of each resident
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication
- Supervise and evaluate CNAs, providing feedback and coaching to ensure they are meeting our high standards
- Participate in fall-risk and pressure ulcer prevention programs, actively working to prevent and manage these conditions
- Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary
- Active license to practice as an LPN
- Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred
