Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse - LPN to join our team at Consulate Health Care. As an LPN, you will play a crucial role in providing high-quality nursing care to our residents. You will have the opportunity to collaborate with our healthcare team to implement individualized treatment plans for each assigned resident, ensuring their unique needs are satisfied.

Responsibilities
  • Provide direct patient care to residents, including administering medications, performing treatments, and monitoring vital signs.
  • Collaborate with the healthcare team to develop and implement individualized care plans for each resident.
  • Supervise and evaluate Certified Nursing Assistants (CNAs) to ensure they are delivering care according to our high standards.
  • Oversee fall-risk and pressure ulcer prevention programs, and actively work to prevent and manage these conditions.
  • Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary.
Requirements
  • Active license to practice as an LPN.
  • Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
