Senior Case Manager

Community Housing Innovations (CHI) is a not-for-profit housing and human services organization that aims to support social and economic independence. We are seeking a highly skilled and experienced Senior Case Manager to join our team in Middle Country, NY.
Job Summary:The Senior Case Manager will be responsible for supervising the overall running of the Case Managers for their site, ensuring quality housing services are provided effectively and in accordance with guidelines established by CHI, OTDA, Suffolk County Department of Social Services, and other funding sources as appropriate.
Key Responsibilities:- Provide crisis intervention as appropriate.
- Support direct case management duties as needed.
- Provide regular supervision for Case Managers.
- Conduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly).
- Ensure that the program is providing quality services and is compliant with program requirements.
- Promote sound social work practices through supervision including case consultations, case file reviews, and employee performance appraisals.
- Ensure the timely and correct submission of all internal and external reports in the manner required by funding agencies and/or CHI.
- Education: BSW or MSW from an accredited university or related field of study required.
- Experience: 2-5 years' experience providing management services preferred.
- Certificates or Licenses: Valid driver's license.
- Communication: Excellent verbal and written communication skills. Proficiency in English required. Bi-lingual (Spanish) is a plus.
- Computer Skills: Basic competency in MS Windows, MS Office, and internet usage. Knowledge of Foothold Awards System a plus.
- Math Skills: Basic math skills required.
- Physical Performance: Ability to tour properties, walk distances, climb stairs, etc.
- Reasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls.
- Other Skills: Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details.
- Salary: $62,400 annually. Exempt.
- Open shifts: [Sunday-Thursday, 11 am - 7 pm]
