ROLE OVERVIEW

The Pathway of Hope Case Manager is responsible for overseeing case management services aimed at assisting families within the community. This role involves implementing program goals through direct service delivery, meticulous record-keeping, and resource identification.

KEY RESPONSIBILITIES

  • Administer case management services in alignment with the Pathway of Hope framework.
  • Conduct intake and assessment processes according to established program standards.
  • Provide tailored services to families, including the development of goals and actionable plans.
  • Work collaboratively with both internal teams and external organizations to facilitate participant success.
  • Track participant progress and ensure comprehensive documentation is maintained.
  • Engage in program planning and enhancement initiatives.
  • Build and sustain partnerships with community organizations to ensure holistic support for participants.

PROGRAM MONITORING

  1. Guarantee the accuracy of databases and assist in the effective implementation of programs.
  2. Contribute to committees focused on case management and emergency support services.

QUALIFICATIONS

  • Bachelor's degree in human services; a Bachelor of Social Work (BSW) is preferred.
  • A minimum of two years' experience in case management or social service roles.
  • Possession of a valid driver's license and completion of required caseworker certification.
  • Proficiency in Spanish is essential.
