Role Responsibilities

The Rehabilitation Case Manager (CM) is responsible for ensuring high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. The CM will coordinate and direct care tailored to the specific needs of each rehabilitation patient. Collaboration with the Rehabilitation Program Director and Facility Case Management Director is essential for effective departmental operations.

Key Responsibilities Include:

  • Providing orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and criteria for continued stay and discharge. This includes information on Medicare and insurance benefits, grievance procedures, treatment planning, and patient rights.
  • Serving as the primary coordinator for patient, family, and caregiver education, encouraging their involvement in discussions about care plans, goals, and progress through Family Conferences and other interactions.
  • Ensuring the execution of the patient's treatment plan that aligns with their strengths, needs, and preferences, while facilitating their active participation throughout the rehabilitation process.
  • Documenting findings from Discharge Planning Evaluations (DPE) and psychosocial assessments, and communicating the social, financial, or discharge needs and preferences of the patient and their family.
  • Promoting consistent, positive interactions with patients that reflect our commitment to exceptional patient service.

Qualifications Required:

  • Registered Nurse (RN)
  • Associate Degree in Nursing
