Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at WES Health System. As a Blended Enhanced Targeted Case Manager, you will play a critical role in supporting individuals with complex needs, connecting them with essential resources, and advocating for their well-being.

Key Responsibilities
  • Assessment and Service Planning: Conduct thorough assessments of clients' needs, develop personalized service plans, and coordinate with community resources to ensure seamless support.
  • Advocacy and Support: Provide emotional support, advocacy, and guidance to clients and their families, helping them navigate the healthcare system and access necessary services.
  • Community Linkage: Establish and maintain relationships with community providers, ensuring clients receive comprehensive care and support.
  • Documentation and Reporting: Maintain accurate records, track client progress, and submit reports as required.
  • On-Call Responsibilities: Participate in a 24/7 on-call rotation, providing emergency support and crisis intervention as needed.
Requirements
  • Education: Bachelor's or Master's degree in Human Services or a related field.
  • Experience: Minimum 2 years of experience in case management, social work, or a related field.
  • Skills: Excellent communication, interpersonal, and problem-solving skills; ability to work independently and as part of a team.
