The Case Manager at Chester Community Coalition (CCC) works to provide blended case management to gunshot victims within the Chester, PA and Delaware County community. Case management services are also provided to community members impacted by intentional violence, including but not limited to gun violence or stabbings.

Case managers focus on connecting clients to resources, supporting clients in meeting their goals to mitigate the impact of violent trauma on their lives, and maximizing satisfaction with the outcomes of their participation in Chester Community Coalition programs. The position is full-time.

SPECIFIC EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES:
  • Experience providing support services for victims of violence with a trauma-informed approach
  • Effective verbal and written communication skills
  • Familiarity with diverse populations and local community resources
  • Familiarity with hospital interdisciplinary teams or Violence Intervention Programming highly encouraged

ESSENTIAL DUTIES/RESPONSIBILITIES:
  • Assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring staff performance.
  • Complete intake interviews; determining need for medical, psycho-social, and psychiatric treatment; reviewing therapist evaluations, treatment objectives, and plans.
  • Set schedules and routines; coordinate services being provided; arranging resources, including transportation and escort.
  • Monitor cases, verify clients’ attendance; observe and evaluate treatments and responses; advocate for needed services and entitlements; obtain additional resources; provide personal support.
  • Maintain client records, review case notes; log events and progress.

  • Communicate client progress in interdisciplinary meetings and supervision; disseminate results and obstacles to therapeutic team and family; identifying treatment influences.
  • Prepare client discharge; review and amplify discharge plans; coordinate discharge and post-discharge requirements; orient and prepare family members; providing resources.
  • Balance work requirements with learning opportunities; evaluate the application of learning to changes in treatment results.
  • Improve treatment results by studying, evaluating, and redesigning processes; implementing changes; rewriting policies and procedures.
  • Meet the budget by monitoring expenses; implementing cost-saving actions.
  • Prepare reports by collecting, analyzing, and summarizing treatment and results data and trends; support grant and subsidy applications.
  • Enhance department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

Education, Experience, and Licensing Requirements:

  • Associate degree; BA or BS preferred
  • Bilingual (Spanish-speaking) candidates are welcome to apply
  • Must have reliable car transportation
  • FBI fingerprint, criminal & child abuse background check
  • Previous case management experience preferred
  • 3+ years customer service experience, preferably in human services, possess demonstrated commitment to customer service
  • Must have a valid driver’s license
  • Must be over 21 years old
