We are currently seeking dedicated Intake Coordinators for full-time positions. Ideal candidates will possess strong interpersonal skills, effective communication abilities, organizational proficiency, and the capacity to work independently to manage daily responsibilities.
Position Summary:
This entry-level role involves comprehensive coordination of care for patients receiving home infusion therapy. Each new hire will be paired with a Senior Intake Coordinator who will serve as a mentor throughout the initial 180-day probationary period.

Key Responsibilities:
- Compile and distribute initial Patient Profiles for therapy to the healthcare team.
- Manage incoming calls for the Intake Workgroup, ensuring proper documentation.
- Communicate effectively with pharmacy and delivery staff regarding patient referrals and issues.
- Collaborate with the Liaison/Care Coordination team to facilitate efficient patient discharge.
- Document all patient-related information in clinical records accurately.
- Update patient files based on daily reports and changes in supply requirements.
- Maintain professional appearance standards as set by NELC.
Qualifications:
- Associate of Arts degree preferred.
- Previous experience in a medical office is advantageous.
- Familiarity with medical terminology is preferred.
