Housing Support Program Coordinator

Carastar Health is in search of a dedicated and experienced professional to oversee our HUD Supportive Housing Program (SHP). As the Housing Support Program Coordinator, you will play a crucial role in leading a team of case managers who deliver vital supportive services to our clients.

Key Responsibilities:
  • Assessing and confirming eligibility for Supportive Housing and Residential Services.
  • Engaging with the community and providing essential information about program guidelines and procedures.
  • Accurately completing referral documentation.
  • Managing rent collection, maintaining financial records, and preparing necessary vouchers.
  • Connecting individuals with appropriate services as required.
  • Overseeing equipment and administrative documentation.
  • Facilitating staff meetings and preparing reports for the Residential Director.
  • Recording consumer information in Electronic Health Record systems.
  • Collaborating with various agencies and service providers for consumer assistance.
  • Guiding and supporting case managers in fulfilling program objectives.
Requirements and Qualifications:
  • Understanding of mental health challenges and behavioral traits.
  • Experience in delivering counseling and support services.
  • Proficient in maintaining accurate records and documentation.
  • Certification as a HUD Housing Quality Standards Inspector.
  • Bachelor’s degree with relevant experience; Master’s degree is preferred.
  • Familiarity with HUD programs is advantageous.
  • Possession of a valid driver’s license and reliable transportation.
