Onsite Hospice Registered Nurse Case Manager

We are seeking a dedicated Onsite Hospice RN Case Manager to join our team, offering a competitive sign-on bonus and a comprehensive benefits package.
Compensation:
Salary range: $100,000 - $115,000 annually

Job Responsibilities:
The successful candidate will be responsible for:
- Managing care for four hospice patients daily
- Conducting both physical and psychosocial assessments
- Documenting patient visits electronically
- Developing and implementing individualized care plans
- Coordinating with physicians, social workers, and spiritual advisors
Work Schedule:
This position requires a Monday through Friday schedule from 8:30 AM to 5:00 PM, with one weekend shift per month.
