Job Responsibilities:
We are seeking a qualified RN with either an Associate's or Bachelor's degree and a minimum of two years of experience in hospice, palliative care, or related nursing fields such as medical/surgical, community health, or oncology.
Work Environment:
This position is based onsite at a skilled nursing facility, where the RN will care for four hospice patients daily. After patient visits, the RN will complete charting and follow-up communications from a home office.
Key Duties:
- Conduct both physical and psychosocial assessments of patients requiring hospice or palliative care.
- Document visit occurrences electronically.
- Develop and implement nursing goals of care tailored to the needs of terminal patients.
- Collaborate with physicians, social workers, and spiritual advisors for comprehensive patient care.
Schedule:
The work schedule is Monday through Friday from 8:30 AM to 5 PM, with one weekend shift required each month.
