Job Responsibilities:
We are seeking a qualified RN with an AAS or BSN degree and a minimum of two years of experience in hospice, palliative care, or related nursing fields. The RN will be responsible for:
- Conducting assessments of patients requiring hospice care
- Managing a caseload of four hospice patients daily
- Documenting patient visits electronically
- Collaborating with physicians, social workers, and spiritual advisors
- Developing and implementing individualized care plans
Work Schedule:
The position requires availability Monday through Friday from 8:30 AM to 5 PM, with one weekend shift per month.
