Position Overview

The Service Coordinator I is responsible for delivering comprehensive case management services, utilizing a recovery-oriented approach that integrates mental health support, substance use services, and community resources for an assigned group of clients. Key focus areas include:

· Blended Case Management: (Behavioral Health)

o Target Population: Deliver case management services to adults diagnosed with serious mental illnesses and children/adolescents experiencing mental health challenges or significant emotional disturbances.

· Erie's Public Schools Case Management: (Student and Family Support)

o Target Population: Provide case management services for students and families within Erie's Public Schools, operating from both school facilities and UPMC Western Behavioral Health at Safe Harbor.

Incentive Opportunity:

Sign-On Bonus Details:

  • $3,000 for a commitment of 18 months

Key Responsibilities:

  • Provide holistic case management to an assigned group of individuals, employing a strengths-based and person-centered recovery model.
  • Exemplify the mission and values of the organization in daily operations.
  • Maintain a flexible schedule to accommodate the needs of clients and the program.
  • Participate in on-call support for after-hours assistance and advocacy.
  • Utilize electronic health records effectively while adhering to HIPAA and privacy regulations.
  • Document all client interactions and ensure timely uploads of notes.
  • Conduct assessments to develop individualized recovery plans addressing various life domains, including mental health, substance use, medical, economic, housing, and vocational needs.
  • Facilitate referrals to community resources and programs tailored to client needs.
  • Monitor service effectiveness and explore additional resources as necessary.
  • Assist clients in creating Safety Crisis/Relapse Plans, identifying triggers and supportive measures during crises.
  • Advocate for clients during hospitalization processes, including assessments, discharge planning, and aftercare follow-ups.
  • Provide crisis intervention services as needed.
  • Engage in quality assurance and improvement initiatives.
  • Foster positive relationships with clients, colleagues, and community organizations to enhance overall service delivery.

Qualifications:

  • Bachelor's Degree in a Human Services field such as Social Work, Psychology, or Counseling is required.
  • Other relevant fields may be considered with a minimum of 12 credits in Human Services coursework.
  • One year of related experience is preferred, especially in community settings.
  • A valid driver's license and a safe driving record are required.
