We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare Home Care. As a key member of our home health team, you will play a vital role in delivering high-quality patient care and support to our clients in the comfort of their own homes.
Key Responsibilities- Oversee the implementation and ongoing assessment of patient care plans through management of home health aides, LPNs, RNs, and other caregivers.
- Ensure accurate evaluations and develop comprehensive care plans that address patient needs.
- Communicate patient conditions and collaborate with healthcare providers to deliver care when patient needs evolve.
- Provide education to patients and families on proper home health care procedures, including wound care, IV administration, medication management, and more.
- Work to decrease readmissions by promoting preventative care and ensuring continuity of care.
- Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required.
- Current CPR/AED/BLS/First Aid certification.
- Reliable transportation to/from care sites and/or work locations.
- One (1) year of professional experience practicing as a Registered Nurse (RN) in a home health setting.
