  • Service Coordination: Evaluate, identify, secure, and oversee services tailored to the unique needs of program participants.
  • Project Management: Work alongside the Project Manager to facilitate the development and execution of innovative initiatives.
  • Participant Engagement: Address participant needs through various communication channels, ensuring attentiveness to their requirements. Assist participants in accessing necessary adaptations, medical equipment, transportation, and additional services.
  • Compliance and Record-Keeping: Adhere to compliance standards and timelines for assessments, service plans, visits, communications, and incident reporting. Maintain detailed and person-centered documentation for all interactions.

Qualifications:

  • Education: A Bachelor’s degree in sociology, social work, psychology, gerontology, criminal justice, or related fields is preferred.
  • Experience: A minimum of three years in case management, particularly within the long-term living sector, is preferred.
  • Skills: Strong communication, customer service orientation, integrity, job knowledge, reliability, problem-solving abilities, interpersonal skills, productivity, quality focus, self-improvement, and teamwork.
  • Technical Proficiency: Competent in utilizing relevant computer systems and technology.
  • Physical Requirements: Capability to perform tasks involving reaching, stretching, stooping, sitting, walking, and standing. Must be able to lift up to 20 pounds independently.
  • Licensure: A valid driver’s license and reliable transportation are required for travel.
