The Service Coordinator I will deliver comprehensive case management services, employing a recovery-oriented approach that integrates mental health support, substance use services, and community resources for an assigned client group. Key focus areas include:
· Blended Case Management: (Behavioral Health)
o Target Audience: Provide case management services to adults diagnosed with serious mental health conditions and children/adolescents experiencing mental health challenges or significant emotional disturbances.
· Case Management for Public School Students: (Student and Family Support)
o Target Audience: Deliver case management services for students enrolled in public schools and their families, collaborating from both school settings and community health resources.

Sign-On Bonus Details:
- $3,000 for a commitment of 18 months
Key Responsibilities:
- Provide thorough case management to an assigned group of individuals, utilizing a strengths-based and person-centered recovery framework.
- Exemplify the agency's mission and values in daily operations.
- Maintain a flexible schedule to accommodate the needs of clients and the program.
- Participate in on-call duties for after-hours support and advocacy.
- Utilize electronic health records effectively while adhering to HIPAA and privacy regulations.
- Document all client interactions and ensure timely uploading of notes.
- Conduct regular assessments to develop personalized recovery plans, addressing various life domains such as mental health, substance use, medical needs, economic stability, housing, and vocational goals.
- Facilitate referrals to community resources and programs tailored to the needs of clients.
- Monitor service effectiveness and assess whether current resources are meeting client needs.
- Assist clients in creating Safety Crisis/Relapse Plans, encouraging them to identify triggers and helpful strategies during crises.
- Advocate for clients during hospitalization processes, including assessments, discharge planning, and aftercare referrals.
- Provide crisis intervention services as needed.
- Engage in quality assurance and improvement initiatives.
- Foster positive relationships with clients, colleagues, and community organizations to build trust and rapport.
Qualifications:
- Bachelor's Degree in a Human Services discipline such as Social Work, Psychology, or Counseling is required.
- Other relevant fields may be considered with a minimum of 12 credits in Human Services coursework.
- One year of related experience is preferred, particularly in community settings.
- A valid driving record is required.
