The Case Management Specialist I will deliver comprehensive case management services, utilizing a recovery-oriented approach that integrates mental health care, substance use services, and community resources for an assigned group of clients. Our primary focus areas include:

· Blended Case Management: (Behavioral Health)

o Target Audience: Deliver case management services to adults diagnosed with serious mental illnesses and children/adolescents experiencing mental health challenges or significant emotional disturbances.

· School-Based Case Management: (Student and Family Support)

o Target Audience: Provide case management services for students and their families within the public school system, collaborating closely with educational institutions and behavioral health services.

Sign-On Bonus Details:

  • $3,000 for a commitment of 18 months of employment.

Key Responsibilities:

  • Deliver thorough case management to an assigned client group, employing a strengths-based and person-centered recovery framework.
  • Exemplify the mission and values of our organization in all daily interactions.
  • Maintain a flexible work schedule to accommodate the needs of clients and the program.
  • Participate in on-call support for after-hours advocacy.
  • Utilize our electronic health record system effectively while adhering to all HIPAA and privacy regulations.
  • Document all client interactions and ensure timely updates of records.
  • Conduct regular assessments to create individualized recovery plans addressing various life domains, including mental health, substance use, medical needs, and vocational aspirations.
  • Facilitate access to community resources and programs tailored to client needs.
  • Monitor service effectiveness and explore additional resources as necessary.
  • Assist clients in developing Safety Crisis/Relapse Plans, encouraging them to identify triggers and supportive strategies during crises.
  • Advocate for clients during hospitalization processes, assisting with assessments, discharge planning, and aftercare referrals.
  • Provide crisis intervention services as needed.
  • Engage in quality assurance and improvement initiatives.
  • Foster positive relationships with clients, colleagues, and community partners to enhance service delivery.

Qualifications:

  • Bachelor's Degree in a Human Services discipline such as Social Work, Psychology, or Counseling is required.
  • Other relevant fields may be considered with a minimum of 12 credits in Human Services coursework.
  • One year of related experience is preferred, particularly in community settings.
  • A valid driver's license and a safe driving record are required.
