Hospice Care Specialist

Job Summary:
We are seeking a compassionate and skilled Hospice Care Specialist to join our team at Advocate Health. As a Hospice Care Specialist, you will provide end-of-life care to patients and their families, ensuring their physical, emotional, and spiritual needs are met.
Responsibilities:
- Provide comprehensive care to patients with terminal illnesses, including pain management, symptom control, and emotional support.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct regular assessments to identify patient needs and make recommendations for care.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Document patient information accurately and thoroughly in the electronic health record.
Requirements:
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) certification.
- Valid driver's license.
- 1-2 years of experience in hospice care or a related field.

- Competitive salary range: $74,672.00 - $112,112.00 per year.
